Our Immunization Schedule (Guideline)

Many new parents often ask what is expected at their child(ren)’s next appointment and ask for a copy of our practice’s immunization schedule.  This is a standard immunization schedule for children.  We know there are many families who would prefer an alternate schedule and we are happy to speak with you about any variations or beliefs that may not follow this standard.  We are open to your preferences.

Like all families, we are concerned about how COVID 19 will affect our loved ones and our community.  Stay up to date on all the latest news by checking out this link with the American Academy of Pediatrics. We are always here to test children with a rapid (15 minute) or a PCR (send out that takes 2-3 days) for results.

Medical Forms and Immunization Records

Request for medical records must be made in writing and contain the signature of a parent or guardian. Medical records requested for personal use will incur a charge of $15. There is not a charge to send medical records to another physician. FMLA forms will be completed for a charge of $25. School and camp physical forms are completed free of charge at the well child visit. There is a $5 fee for forms completed any time other than at the well child exam as long as the patient has had a well child visit within the past 12 months. Please allow 48 to 72 hours for the medical records request. These requests must be picked up at our office.
